Abstract Aims A mesenteric cyst, rare intra-abdominal pathology, poses a clinical challenge due to lack of pathognomonic symptoms, variability in presentation and location. We present an interesting case with significant diagnostic dilemma; suspected benign ovarian cyst invading the mesentery on repeat imaging laparoscopy is questioned for origin. Methods 77-year-old female presented 6-year history abdominal mass. She had multiple radiological scans which reported it as simple but origin could not be defined. repeated aspirations cytology. They developed septations, filled-up rapidly, became from pelvis. careful decision was taken surgery. The procedure undertook general surgeons gynaecologists possibility cyst. During laparotomy, found extending pelvis base up diaphragm densely adhered sigmoid colon. completely excised hysterectomy oophorectomy. Results patient recovered well Post-operatively, histopathology confirmed Figo IC2 Grade 2 mucinous adenocarcinoma ovary expansive growth pattern. awaiting further treatment chemotherapy after discussion Gynaecology MDT.
